Exams
BAMS have arranged the following dates for special visit exams to take place at the Bristol Centre for Music and the Arts, Southmead:
Trinity Guildhall: Monday 9, Tuesday 10 and Wednesday 11 July 2012, (and possibly 12 July if required).
ABRSM: Monday 16, Tuesday 17 and Wednesday 18 July 2012.
In order to enter for exams on these dates parents must complete the relevant letter (attached below) and return with payment to the BAMS office by Monday 28 May. BAMS can arrange an accompanist if necessary.

Lesson Plans
The Termly Lesson Plan and Record needs to be completed for each group of pupils taught (excluding Wider Opps).

The Lesson Plan needs to be completed before any BAMS lesson observation or if Ofsted are visiting a school on the day you teach. BAMS will inform you in advance of either of these events.

Lesson Observations
BAMS will Lesson Observe all tutors on a regular basis. Below is the sheet that will be used during the observation, and a copy of the FMS Standards that will be applied. All staff will be required to attend a follow up meeting after their observation to agree a plan of action.

Record of Hours forms
Record of Hours forms must be completed by all tutors at the end of each month. They need to be returned to the BAMS office within 5 working days of the following month. They can be submitted electronically or by post. Any additional payments due to staff will be calculated from these form. Charges to schools are also based on this data.

Monthly Travel claim
Monthly travel claims must be submitted within 3 working days of the month following your claim. They must be submitted in the post. Tutors must attach VAT receipts for fuel with each claim (approximately £40 for every 100 miles claimed).
Staff can only claim for miles travelled within Bristol boundaries. All miles must be split into Business miles and Call Out miles on the claim form. Business miles relate to any journeys undertaken during the working day between schools. Call Out miles relate to your first and final journey of the day (ie from home/Bristol boundary to first school + last school to home/Bristol boundary).

Timetable Changes form
Schools cannot decrease the length of their booking during a double term. The amount of time tutors work at a school can be increased during a double term if necessary. In order to action any increase tutors must complete a Timetable Changes form and get it signed by a relevant member of staff at the school. The completed form must be submitted to the BAMS office. Any payments due to staff for additional hours cannot be authorised without a completed Timetable Changes form.

Timetable Update form
BAMS tutors are asked to complete a Timetable Update form after the first few weeks of each double term. This allows the BAMS management team to keep an accurate record of where staff are working, and when.

Register returns
BAMS tutors must complete a register for all peripatetic tuition (small group). Tutors will require one register per school. Completed registers must be returned to the office at the end of each double term.
Tutors delivering Wider Opportunities projects or whole class tuition must also submit a register showing the names of all students taking part.

Evaluation data must be collected at the end of each Wider Opportunities project. Forms will be sent to tutors towards the end of each project of can be downloaded here:

Exam Record
BAMS must collect details of all pupils entered for exams by our tutors. This form should be completed when necessary and returned to the BAMS office. The data will be used for statistical purposes.

Absence Self certification form
This form should be submitted whenever you are absent from work. If your absence exceeds 5 working days then a Doctors note must also be submitted.
