Transition Information Project

The project - a partnership between Children Adults & Families Directorate, Connexions and other partners - helps young people with additional needs and disabilities in the “transition” to adult life and services.

It helps young people and their parents and carers to make important choices around things like:

• Education and Training
• Work
• Housing
• Sports and Leisure
• Health
• Benefits
