Lost Blue Badge
There is a £10 fee to replace a lost badge.
If your Blue Badge has been lost you'll need to download the pdf Blue Badge replacement form (128 KB) , fill it in and post it to us.
Stolen Blue Badge
There isn't a fee to replace a stolen Blue Badge.
If your Blue Badge has been stolen you'll need:
- to download the pdf Blue Badge replacement form (128 KB) , fill it in and post it to us

Damaged Blue Badge
There is a £10 fee to change a name, and replace a damaged or illegible badge.
Download the pdf Blue Badge replacement form (128 KB) , fill it in and post it to us, along with the damaged badge.
We can also send you the form by post. Contact us to ask for a printed copy.
