Adult care referral form for professionals

Use the adult care referral form for professionals to:

  • refer an adult for a needs assessment
  • tell us about a change of needs
  • provide additional information for a referral already made
  • check the progress of a referral already made
  • tell us about a change of circumstances for someone who is receiving direct payment (direct payment support service only)

If you're not a professional, and you want to refer yourself, a relative, friend or neighbour, you'll need to use the adult care referral form for the public

Safeguarding form for professionals

Use the safeguarding form for professionals if you suspect an adult in Bristol with care and support needs is:

  • at risk of or experiencing harm from others due to abuse or neglect
  • at risk of self-neglect but their health or welfare is not in immediate danger
  • unable to protect themselves from harm because of these needs
Occupational therapy referral for professionals

Use the occupational therapy referral form for professionals to refer adults who have a long term disability which directly affects their ability to manage at home.

If you're not a professional, use the occupational therapy referral form for the public.

 

 

Discharge to Assess referral

Use the Discharge to Assess referral form if you need to refer a service user who is:

  • currently getting a service from a Discharge to Assess pathway (Pathway 1, 2 or 3) 
  • being supported by Urgent Care response to Bristol City Council Discharge to Assess team for an assessment of their care and support needs
Continuing Health Care assessment

Use the Continuing Health Care (CHC) assessment form:

  • to make a new CHC assessment
  • for a CHC review
  • to end CHC funding
Refer an adult who is currently in hospital or a mental health hospital
Request a Social Circumstances Report for the Mental Health Review Tribunal
Refer an adult who is currently awaiting prison release
Refer a child to the Young Adults Transitions Service

Our Young Adults Transition Service (YATS) supports young people to move from Children's Services to Adult Social Care, if that's what the young person needs.  

YATS work collaboratively with:

  • Children Services
  • schools and colleges
  • the NHS 
  • the team responsible for education, health and care plans (SEND)

We aim to support young people in their transition to adulthood, based on the four pillars of the Preparing for Adulthood principles:

  • Education, training, and employment
  • Independent living
  • Community inclusion, having friends, and being part of the community
  • Being healthy

The information you provide us in this form will be used to:

  • determine if YATS is the right pathway for the young person
  • provide early support, advice, information and signposting as appropriate
